REFERRALS:
Dr Bullard is a specialist. Some insurances (ex: HMO’s that are not “Open Access”) require a referral from the Primary Care Physician (PCP) in order for the patient to be seen by a specialist (i.e., Dr. Bullard). This is an insurance requirement. It is the patient’s responsibility to ensure that this referral is generated by the PCP and sent to Dr. Bullard’s office prior to the appointment. If the referral is not received by Dr. Bullard’s office, the insurance will not cover the cost of the exam. Please check with your insurance prior to scheduling the appointment to see if you are required to have a referral to see Dr. Bullard.
MINORS:
If the patient is under 18 years of age, a parent or legal guardian must accompany the patient to their appointment. If a parent/legal guardian is not able to accompany the patient, the parent/legal guardian must complete all paperwork ahead of time including the authorization for non-parent/legal guardian to accompany patient form authorizing another adult to bring their child to the appointment. In addition, the accompanying adult must be authorized on the HIPAA form.
PAYMENTS:
Dr. Bullard accepts cash, check, Visa and Mastercard. All copays, deductibles, and/or coinsurances are collected at the time of service. In addition, the cost of any services deemed “non-covered” by the insurance will also be collected at that time. Office policy requires that we keep a valid credit card (Visa or Mastercard) on file for each patient; regardless of the chosen method of payment please bring a valid credit card (Visa or Mastercard) to the appointment for this purpose in addition to completing the Financial Policies form.
WHAT TO BRING TO YOUR APPOINTMENT:
- ID card for the patient (or parent/legal guardian if a minor)
- Medical insurance card
- Completed paperwork
- Any records from previous ophthalmologists
- Referral from primary care doctor (if required by insurance)
- Glasses and/or contact lenses (including the box that the contact lenses came in)
